In regards to your clinical question, QL/low back, hip external rotator muscle and sciatica issues occurring together is something I see in the clinic on a fairly regular basis. In my mind, much of this probably has to do with the origin of the sciatic nerve (branches in the lumbar spine) and its path through the posterior hip region. I often find that a combination of spinal facet joint mobilizations, soft tissue work through the lumbar spine (including the QL) and the posterior hip region and exercises (hip and lumbar mobility and nerve mobilizations) help with these symptoms. You might find the exercises in the piriformis/deep gluteal pain syndrome program here in the app to be helpful. Here is a link to that program.

Hi Shaf, It's great to meet you and learn about your clinical background! The combination of massage therapy and personal training is such a good one. I would imagine you are quite similar to me in that I also combine manual therapy and exercise. Thank you for joining my app and community. Regarding your questions, if you look at the top of the home page just below the featured section, you will see a search bar on the left and a 'filters' area on the right. If you click on filters, there is an area where you can search by muscle group. I need to build this out more, but this may help with some of your searches. I have attached a screenshot where you can see the filter area across from the search bar.

Hi Anagha, It's great to learn more about you! Your work sounds very interesting. I had considered being an Air Force physical therapist when I first graduated from my DPT program, so it's cool to learn that you are doing similar work.

Yes, the training is different between Australia physios and US physical therapists and I’m not entirely sure if you have vestibular specialist physios in Australia. Did your client see an audiologist for their ear testing? Are there any neck movements or positions that change their ear symptoms? I have seen cases like this in the past where a neck injury was associated with cranial nerve changes, so it’s definitely possible that the neck symptoms are associated with the ear symptoms.

Hi Cathy, it’s so great to meet you! Thank you very much for purchasing a copy of my book. My series of seven smaller body region specific books will come out as a group in May of 2026. They will be quite similar to the body region chapters in the main book, but some people wanted cheaper books that were specific to only one body region, which is why I created them.

Hi Caroline, My apologies for not responding earlier. For some reason, I didn't get a notification about this message. 

To be honest, I'm not sure about your client's symptoms as they are rather unusual. While the cervical nerves can sometimes interfere with areas innervated by the cranial nerves (such as hearing) this is rather uncommon. Does your client have any other symptoms that may be associated with ear dysfunction, such as dizziness or vertigo? Has this client been assessed by a vestibular physical therapist? They are very good at assessing the inner ear and neck at the same time and may be able to identify if the two are related.
